  • Title: Colchicine in Haemorrhagic Stroke Patients with Complicated Diabetes Mellitus: A Systematic Review on Efficacy and Safety

Abstract

Aims and Objectives:

● To assess the efficacy of colchicine in improving clinical outcomes, as measured by aDCSI score, in hemorrhagic stroke patients with complicated diabetes mellitus.

● To review the effects of colchicine on incidence rates and hazard ratios of hemorrhagic stroke.

Background:

This systematic review investigates the relationship between colchicine use and the risk of hemorrhagic stroke in patients with complicated diabetes mellitus (DM). DM is a chronic medical condition that can lead to various complications, including cardiovascular diseases such as stroke [1]. Hemorrhagic stroke is a severe subtype of stroke that can result in significant morbidity and mortality[2]. Colchicine, a medication commonly used to treat gout and other inflammatory conditions, has been shown to have potential benefits in reducing the risk of hemorrhagic stroke [3]. This study aims to investigate this further and evaluate the effectiveness of colchicine as a treatment option in reducing the risk of hemorrhagic stroke in patients with complicated DM.

Rationale:

Studies have revealed the potential advantages of Colchicine, a low-risk and cost-effective medication in reducing the occurrence of cardiovascular illnesses such as Ischemic Stroke [4]. This is the first known systematic review to examine the drug's effectiveness in treating hemorrhagic stroke in a given patient population.

Methodology:

This systematic review utilized the Preferred Reporting Standard of Systematic Reviews and Meta-Analysis (PRISMA) standard and checklist to ensure precision in the search, screening, and data extraction. A thorough literature search was conducted in 3 standard databases using the following search keyword strategy:

Quality assessment of the study will be done using the Newcastle Ottawa scale. The systematic review is registered with the International Prospective Register of Systematic Reviews (PROSPERO), with reference ID CRD42023400819.

Results and Interpretation:

Despite the initial identification of 60 records, only one study met the inclusion criteria after removing duplicates, editorials, reviews, and inaccessible studies. This study demonstrates that colchicine was associated with a lower risk of hemorrhagic stroke in DM patients. The adjusted hazard ratio of the colchicine cohort was 0.66, with a 95% confidence interval of 0.53-0.82. However, the subdistribution hazard model indicated no association between colchicine and hemorrhagic stroke in DM patients without gout. Although limited by a small sample size and including comorbidities, this study provides valuable insights into colchicine use in treating hemorrhagic stroke and calls for further research to evaluate its efficacy and safety fully.
