Abstract

Alzheimer’s disease continues to be described using broad clinical labels such as mild, moderate, and severe despite significant variability in cognitive, behavioral, emotional, and functional presentation across disease progression. These generalized terms lack measurable specificity, limit interdisciplinary communication, and often fail to align neurological change with real-world functioning and care needs. This presentation introduces the Alzheimer’s Tiered Progression Matrix (Pro-Ma™), a structured functional framework designed to improve staging precision and clinical communication in Alzheimer’s disease. The framework organizes progression into three neuro-functional phases: Neuroamnestic, Neuroadverse, and Neuroabsentia, paired with dynamic functional tiers identified as Emerging, Sustained, and Transitional. Unlike traditional staging models, the Pro-Ma™ framework emphasizes observable functional presentation, executive decline, behavioral expression, emotional dysregulation, communication changes, and retained abilities across progression. The model also incorporates biomarker-informed understanding alongside clinical observation to support interdisciplinary application across neurology, psychiatry, residential care, and community-based settings. Special attention is given to the Neuroadverse phase, where neuropsychiatric symptoms including paranoia, hallucinations, emotional lability, executive dysfunction, and fluctuating cognition frequently emerge yet remain poorly differentiated within existing staging terminology. As advances in biomarkers and neuroimaging continue to evolve, Alzheimer’s staging language must evolve alongside the science. The Pro-Ma™ framework proposes a more clinically actionable, neurologically aligned, and human-centered model for understanding disease progression and supporting precision-based care planning.
